FOR COMPLEX CASES
When someone needs more than fragmented support
A person may need more clinical intensity alongside an existing therapist, more structure after residential care, or coordinated support across mental health, substance use, neurodevelopmental needs, and physical health.
Harbor Home brings these moving parts into one coherent programme: handpicked clinicians coordinated around the individual, clear responsibilities, regular review, and a defined next step if needs change.
NOT A DIRECTORY OF SERVICES
One consultant. One case manager. One plan.
Harbor Home is multidisciplinary, programme-based and formulation-led. A named consultant provides clinical oversight; a dedicated Case Manager coordinates the programme; and the wider team is built around what the individual actually needs.
Psychiatric, psychological, medical, functional, and wider life circumstances are considered together, so prescribing, therapy, coaching, family work, and other interventions sit within one plan, with regular review and clear communication throughout.
BUILT FOR PROFESSIONAL COLLABORATION
How we work with referrers
Psychiatrists & therapists
Keep your client. Gain a team.
Some clients need more clinical capacity than one practitioner can safely provide alone. Harbor Home adds the wider team and infrastructure while allowing your therapeutic relationship to continue.
Private GPs & concierge medicine
A consultant-led MDT behind your practice.
Where needs have become more complex, we place a consultant-led MDT behind your practice. Medical, psychiatric, and functional needs are considered together, with clear clinical ownership and communication.
Family offices & private client advisers
One team, one point of contact, absolute discretion.
When a principal or family member develops a complex health need, Harbor Home provides one medically led team and one Case Manager, with care designed to flex around the realities of the individual’s life.
Education & university consultants
Whole-person care around study and adult life.
For young adults, difficulties in study can sit within a wider picture involving ADHD or autism, mental health, substance use, burnout, or day-to-day functioning. We bring those strands together within one multidisciplinary programme.
INDICATIONS
When we can help
Several needs are starting to overlap.
Mental health, substance use, neurodevelopmental, physical health, family, or functional concerns may all be affecting one another.
Conventional outpatient support isn’t sufficient.
The individual may need more frequent contact, greater clinical oversight, or care beyond the consulting room.
A clinician needs more infrastructure around the case.
We can add multidisciplinary capacity without replacing established therapeutic relationships.
Care needs to work around real life.
Home, work, education, travel, or family circumstances may make a fixed-location model impractical.
The family or wider system needs support.
Where clinically appropriate and agreed, we can incorporate structured family or relational work.
A clearer safety net is needed.
Planned review points and agreed escalation routes provide a clear next step if needs intensify.

NOT AN APPOINTMENTS SERVICE
Clear clinical boundaries are part of good care
We do not offer:
- Single psychiatry appointments
- Prescription-only or titration-only services
- Standalone therapy hours
- Isolated assessment without next steps
Complex needs rarely improve in isolated hours. So prescribing, therapy, and everything else sit within one programme, held by people who understand the whole picture. If we’re not the right fit, we’ll say so – and help you find who is.
HOW IT WORKS
A clear pathway
You can speak to us before making a formal referral. We’ll help establish whether Harbor Home would be appropriate, and what would happen next.
Where immediate stabilisation is clinically indicated, this can begin alongside the assessment.

FAQs
Yes. Where clinically appropriate and with the individual’s agreement, existing psychiatrists, therapists, GPs, and other professionals can remain involved. Harbor Home is designed to add capacity around established care, not automatically replace it.
Communication is agreed according to the individual’s consent, your professional role, and clinical need. Relevant professionals receive updates at appropriate points through assessment, review, and transition or discharge.
Where Harbor Home is the appropriate route, care begins with multidisciplinary assessment and formulation. This allows the team to understand the whole picture before recommending the programme, intensity, and setting that are clinically appropriate.
You do not need to determine the final setting before referring. If assessment indicates that residential treatment would be safer or more appropriate, we can discuss that pathway.
Yes. A family member, adviser, or professional can begin with a confidential conversation about the situation and possible next steps. Any subsequent clinical engagement is managed through the appropriate consent and assessment process.
No. Neurodevelopmental assessment and treatment sit within a broader whole-person programme where clinically appropriate. Harbor Home does not operate as a standalone assess-and-prescribe or titration-only service.
Harbor Home is an adult service. Where an enquiry concerns someone under 18, we can help identify an appropriate specialist route.
